  • Patient instruction sheet from your referring provider
  • Parent/guardian valid government‑issued photo ID and insurance card (if applicable)
  • Legal documentation if you are not the biological parent
  • Any medications your child is currently taking
  • Radiology images on disc, if appropriate
Recovery & Follow‑Up
After initial treatment, your child will return for follow‑up visits so our specialists can monitor healing and determine a safe timeline for returning to normal activities.
