In the MyChart patient portal, patients can access information regarding outpatient and inpatient clinic visits, including therapy, at any of our U.S.-based locations. Patients and families can also utilize MyChart Bedside from their own phone or tablet during admissions to stay engaged with your care, be empowered with education and clinical data, and better connect with your care team. With proxy access, caregivers and loved ones can also stay informed about a patient's care and communicate with their care team.
When your child is scheduled or registered for an appointment, you will receive an email with an authorization code and login instructions to access MyChart. You can request an activation code at any time via the link below or by calling the MyChart help desk at 855-823-7855 (option 8).
MyChart can be accessed online or through the MyChart app, which can be downloaded from Apple App Store or Google Play Store.
