If your child is a current Shriners Children's patient, we recommend using the Patient Portal for appointment requests and communication with your care team.  The Patient Portal offers additional features, including a date picker for scheduling, reminder notifications, medical records access and more.

If you've never accessed the Patient Portal, you may use the form below to send us an appointment request. Please DO NOT submit multiple appointment request forms. Our system only recognizes one form per patient. Our intake team will review the information and reach out via email and phone as soon as possible. Thank you.
