Shriners Children’s Northern California Hosts Inaugural Pathways to Possibilities Resource Fair

In September, Shriners Children’s Northern California celebrated a milestone with its first-ever Pathways to Possibilities Resource Fair, an empowering event designed to guide young adult patients and their caregivers through the transition to independence.
Focused on promoting confidence, collaboration and long-term success, the fair brought together resources across three key areas of Health Care Transition:
- Medical Health Care – Specialists shared guidance on managing health needs into adulthood.
- Life Skills – Attendees discovered tools for daily living and self-advocacy.
- Higher Education & Vocation – Local programs highlighted pathways to independence through education and career development.
Patients and families connected directly with agencies, support networks and specialists, gaining valuable insights to ensure a smoother transition and enhanced quality of life. Tables of resource lists and brochures for take-away were also available following a one-on-one career assessment with Shriners Children’s Northern California social workers.
