Your referral has been submitted. One of our intake coordinators will review your patient’s information to determine if they are a candidate for care at Shriners Children's. Please allow 3-5 business days to receive a follow up call regarding your referral. Please do not submit more than one form, our system will not recognize multiple submissions.
Following this review, the intake coordinator will contact the patient's family/guardian. If they are unable to reach them by telephone, we will send email and mail communications.
